Rates and Policies

Rates

  • To get started as a new patient it is $400, which includes two comprehensive appointments: an initial 90-minute session and a follow-up visit of up to 60 minutes.

  • After you finish your new patient visits it is $50 for 15 minutes for follow ups and other concerns for all of our services with Dr. Shaanta Hanagud MD & Lynda Kelly​ DSS, LMT 

  • Price determined by nature of the visit & time seen 

  • We take all forms of payment; cash, credit card, check.

Have insurance?

Our practice operates on a fee-for-service basis and does not accept insurance directly. We will gladly furnish a detailed invoice with the appropriate billing codes, which you may submit to your insurance company or health plan for reimbursement. Reimbursement policies vary, so we recommend contacting your provider to confirm the details of your coverage.

 

Important: We are a Medicare opt-out practice, and Medicare will not reimburse for our services."

Appointments

 If Gaia Family Medicine owes you a refund due to an overpayment or credit balance, we will issue a refund after our management has verified it. Provided there are no other balances owed to Gaia Family Medicine, or any of its providers, we will credit your credit card or send you a check, depending on how you made your initial payment. There are no refunds for prepaid packages.

Return Policy for any products

Due to the specific nature of products we offer, we do not accept returns. We do not resell returned items for your protection. No one can guarantee that the storage conditions are met during the order of transportation; thus, we cannot accept them even if the package remains unopened. All supplement sales are final.

Office

Office Retail M-F 11am-6pm

 

Appointments are not required for the purchase of retail products or supplements. We are open to the public. We also welcome walk-ins for inquiries regarding the availability of our wellness services.

 

Office Environment: To ensure the comfort of all patients, we request that you refrain from wearing perfumes or heavy oils to your appointment. Additionally, if you smoke, please ensure a minimum of 30 minutes has passed since your last cigarette/cigar to dissipate any lingering odor.

Cancellation Policy

Everyone's time is valuable, and we understand that unforeseen events can happen that prohibit you from keeping your appointment. We understand this. However, we are a small practice with short windows in order to do business, so it's important to understand that any cancellation undercuts our bottom line, and we cannot absorb these costs like large medical practices.

 

For any reason you cancel your appointment with less than 24 hours' notice, you will be charged a cancellation fee as follows:

  • $25 for Regular Medicine Visit & NRT Recheck Visits

  • $150 for New Patient Visits or Report of Findings Visits.

If you cannot make your appointment, please call or send us a message via the Portal within 24 hours and inform us so other patients have the opportunity to utilize this time.

 

If you are a new patient and do not show up for your initial visit and want to make another appointment a late fee of $150 will be charged as well as payment upfront for that visit.

Late Policy

  • Initial Visit: If you are more than 15 minutes late for your initial visit, we reserve the right to reschedule your appointment, and a $150 missed appointment fee may be charged if we are unable to provide the comprehensive time required for your exam.

 

  • Regular Medicine Visits, Virtuals & NRT Recheck Visits: As these appointments are typically 10 to 15 minutes in duration, there is limited flexibility for late arrivals. A $25 fee will be applied, and we will endeavor to reschedule your appointment for the same day or as soon as possible.
